Shared hosting (cPanel):
1. Log in to cPanel
2. Go to MySQL Databases
3. Create a database user and note the username and password
4. Assign that user to a database with All Privileges

Local (XAMPP / MAMP):

Almost always localhost — do not enter your domain name here
